NEW PATIENT PAPERWORK

FIRST: Please make sure you have scheduled your New Patient Appointment on Schedulicity by clicking HERE.  If you do not see an appointment confirmation email OR a "Thank You" page after you submit the appointment, you haven't finished yet and need to go back to finish.  Then make sure you return here to complete your New Patient Paperwork. 

New Patients must complete and submit the New Patient Paperwork prior to arriving for the first appointment. This is to ensure we receive and review your paperwork prior to your appointment. 

Paperwork is not completed in our office. Paperwork is due at least 24 hours prior to your appointment to avoid cancellation and a cancellation fee. If you arrive to your new patient appointment and have not completed your paperwork, your appointment WILL be rescheduled and you will be charged a fee of $100 (see our 48 hour appointment policy). We make multiple attempts to contact you prior to your appointment to complete the paperwork. Our physicians review your paperwork prior to your appointment; a very important part of your treatment plan. 

In an effort to provide excellent care to our patients, our schedule is kept on time and is strictly structured. This is so we can focus time and attention to each patient in a non-rushed manner.
