SCHOOL & SPORTS PHYSICALS

We offer both limited school/sports physicals or comprehensive physicals in conjunction with regular in-office visits only. 

School & Sports Physicals - LIMITED Examination with completion of forms
EKG Offered for high school (must schedule with in-office APRN)

This is a brief, limited visit that is school/sports physical-focused only. Forms will be completed at the time of the visit.

Annual COMPREHENSIVE Wellness Physical Examinations with completion of forms

Comprehensive Annual Wellness Physical Examinations are not the same as our basic limited school & sports physicals. This is a complete comprehensive & thorough physical examination. Unlike the basic limited school/sports physical (which we also offer), this Annual Wellness Physical Examination will cover any complaints, concerns, symptoms, medical history, etc. We will order any necessary screening or diagnostics labs and complete all necessary physical forms. 

We offer both traditional and holistic healthcare and we are V or Non V friendly & offer exemptions, when medically necessary. 

Most high school Sports Physicals REQUIRE a one-time EKG for grades 9-12. We only offer this EKG in-office when the APRN is in.
