SCHOOL & SPORTS PHYSICALS

WE ARE CURRENTLY BOOKED UP AND SOLD OUT OF THESE PHYSICALS. PLEASE CHECK BACK NEXT SCHOOL YEAR! IF YOU WOULD LIKE YOUR CHILD SEEN, PLEASE SCHEDULE A REGULAR NEW PATIENT VISIT.  SCHOOL & SPORTS PHYSICAL FORMS CAN BE COMPLETED AT THAT VISIT. 

School & Sports Physicals are available now until we are full. 

A sports physical allows the doctor to identify any condition(s) that might keep your child out of the game and affect performance. The sports physical form will be completed during your visit; please bring this with you. PLEASE NOTE THAT AN EKG IS TYPICALLY REQUIRED FOR ALL HIGH SCHOOL SPORTS PHYSICALS (ONE EKG/ONE TIME IS REQUIRED FOR GRADES 9-12).  WE DO NOT CURRENTLY PERFORM EKGs SO PLEASE DO NOT SCHEDULE YOUR HIGH SCHOOLER IF THIS IS NEEDED. REMEMBER, ONLY ONE EKG/ONE TIME IS REQUIRED FOR GRADES 9-12. 

A school is physical may be required for your student to enter a certain school or specific grade level. The school physical form will be completed during your visit. 

School/sports physicals are brief (typically less than 10 minutes). This does not substitute an annual or complete wellness physical. Please note, we do not administer vaccines at our office. We can gladly refer you out for whatever vaccines might be appropriate for your student, if requested. If a vaccine history is needed, we can provide that through the state database. (Our office is pro-vaccine CHOICE so no judgment here!)

Physical $40 -please bring your forms with you.
