The CHARM Patient Portal is required in order to be an active patient at our practice. We understand that it can be frustrating to have portals at every single medical office, but ours is unique. Functional Medicine is totally different in the way we treatment plan, so we share so much information with our patients. It is also the way to reach your provider.
CHARM is HIPAA compliant and secure. It is your ALL-IN-ONE-PLACE portal to do everything in our practice!
For messages, we try to reply to all messages within 1-2 business days. If your need is urgent, please contact your PCP or go to the ER or Urgent Care (Functional Medicine does not manage complex or urgent needs).
You will be able to self-manage your appointments in the portal. You are able to schedule, reschedule or cancel appointments up to 48-hours before your appointment. The system will lock you out from making any appointment changes at the 48-hour mark. We have a very strict 48-hour appointment change policy. To avoid a $100 fee, please ensure all appointments are appropriately & properly scheduled for the correct time, type and provider. Please ensure ALL your results are in before scheduling your appointment.
To activate your portal, please follow the instructions in your "Activation Email" that was sent just prior to or at your new patient visit. It will come from Dawson Integrative Medical Center so there is a chance it went to your spam.
If you have already activated your patient portal, you can access it with the button below. If you get locked out of your portal, you can use the OTP by having a code get sent to your cell phone on file. If you need to change your login completely, you will need to contact the provider to do so.
Login to your Patient Portal prior to appointment time
Go to 'Appointments' section
Click on the 'Check-in' button shown next to appointment
Enter your personal details and join the video session
By clicking the boxes above! You can access it through the web or via apps on your phone. We highly recommend the apps since it has complete functionality.
When you log in on your app, there will be a BOOK NOW button under your Appointment Section. Just follow the instructions to request your appointment. *This is a request. Your appointment will be confirmed once reviewed by the providers.
When you log in on your app, go to the Appointment section and view the appointment. There will be a box with 3 dots next to your appointment. Click on that to CANCEL. There is not an option to reschedule at this time so you will have to CANCEL your original appointment and re-request a new appointment.
*REMEMBER you are not permitted to make any changes to your appointment if you are within a 48-hour window. No appointment changes are permitted without a 48-hour notice.
This must be done through the app. When you log in to the app, click on MY HEALTH tab at the bottom and then under BILLING, click CARD ON FILE to update!
A few ways! When your appointment is confirmed and also during confirmation, you will receive a link. Alternatively, you can log in to your portal and click the CHECK IN button that is in your Appointment Section.
When you log in to your portal, there is a Messaging section. Open & create the message to your provider.
It is best if you message your provider directly with any records to be added to your chart and reviewed. You can attach them to your message. The "document" section is not very patient-friendly and we often do not receive notification that anything has been uploaded or shared.
