The CHARM Patient Portal is required in order to be an active patient at our practice. We understand that it can be frustrating to have portals at every single medical office, but ours is unique. Functional Medicine is totally different in the way we treatment plan, so we share so much information with our patients. It is also the way to reach your provider.
CHARM is HIPAA compliant and secure. This is where your will access your results, orders, visit summaries and treatment plans. This is also the way you will contact your provider with any questions. We try to reply to all messages within 1-2 business days. If your need is urgent, please contact your PCP or go to the ER or Urgent Care (Functional Medicine does not manage complex or urgent needs).
To activate your portal, please follow the instructions in your "Activation Email" that was sent just prior to or at your new patient visit. It will come from Dawson Integrative Medical Center so there is a chance it went to your spam.
Whenever your provider has shared something with you in your portal or sends you a message, you will be notified by email sent to you to check your portal. It is very important you remove from spam and allow the filter so you do not miss important information.
If you have already activated your patient portal, you can access it with the button below. If you get locked out of your portal, you can use the OTP by having a code get sent to your cell phone on file. If you need to change your login completely, you will need to contact the provider to do so.

This is where you will SELF-MANAGE your appointments and who the billing takes place. You are able to schedule, reschedule or cancel appointments up to 48-hours before your appointment. The system will lock you out from making any appointment changes at the 48-hour mark. We have a very strict 48-hour appointment change policy. To avoid a $100 fee, please ensure all appointments are appropriately & properly scheduled for the correct time, type and provider. Please ensure ALL your results are in before scheduling your appointment.
If you need to update your card on file, this must be done while booking an appointment. If you need to make changes to the card after an appointment has already been booked, please message your provider in the secure portal with your new card information.
Our providers are not able to make any scheduling changes on your behalf.
Functional Medicine offices work much differently from traditional medicine. We will strictly enforce the 48-hour policy and we thank you for respecting this.
We value your time & expect the same in return. We are a small practice that spends extended time with our patients. We often reserve 45-60 minutes for appointments & when we are not given the courtesy of a 48-hour notice, we are unable to fill that time with another patient. We are a very small practice so this is the only way we are able to keep our fees so low and offer other cost effective services. There is a $100 fee when a 48-hour notice is not provided for canceled/rescheduled, any appointment changes or if there is a no-show/missed appointment.
